Lady Gaga plays The Countess, who due to an ancient blood virus craves the taste of human blood.

The next scene shows Donovan banging on Ramona Royale’s door. Donovan tells Ramona that she’s drunk. She says that she hasn’t harmed Holden, but rather saved him, as she has saved all her children. Liz says that she wasn’t wearing a costume and explains to him about Devil’s Night. For some reason, Alex believes that injecting him with her blood will heal him, assuming all will be fine. He tells her how he almost died in the hospital.

When John goes back to the room, Aileen is gone. Shortly after being discharged from the hospital, we find Max sucking his parents dry before hopping on the bus to school. Police surround the school. But while waiting for the police to arrive, the blood-crazed children slaughter all the adults and use the downtime to get their story straight – that a tall man in a mask did all the murders.

Back to Alex, she stops by the hotel to see Holden and apparently start her new job as the nanny of the vampire children. The show proceeded, as it ambitiously established further complexity within numerous characters. She is at her office, where she takes her temperature, which reads 75.5 degrees. Who her true love is will finally be unveiled. Since the last time he made an appearance, the boy has gotten a lot worse and has developed pneumonia. Alex insists to the boy’s mother that she won’t give up on him until he is better.

After learning that Holden is still “alive”, Alex attempts to take him home. She heads back into Max’s room and injects a syringe of her own blood into his IV. After he is infused with her blood, he miraculously recovers. Already a somewhat reckless character, as evidenced by her decision to turn at the hands of the Countess (Lady Gaga), she’s faced with a child who is dying of measles because his mother didn’t get him vaccinated.

But in a key flashback viewers saw him creep to the door to fetch room service champagne – then drop the bottle as he turned around in his room to see The Countess staring at him.

Back at the front desk, the new guests call down and order pate. Mischievously, Liz Taylor tells Iris to give them cat food instead. It’s a playful riff on the episode’s (and the season’s) theme of transformation, it’s a suitable origin story for Liz Taylor, and it’s a refreshing break from the predictable, even inevitable plot unwinding in the present-day Cortez. He then remembers having sex with her and tells her that she needs to leave. After the wedding, he took a trip with friends to Los Angeles and stayed in the hotel. Now they could get revenge on the Countess. During this meeting, The Countess tells Liz that he looks and sounds like a man, but he smells like a woman inside. The Countess said Alex should simply say “Hello” to John.

Luckily, The Countess swoops in and slits their throats. The Countess helped him to be free and embrace who he was inside. John can relate to that, and they actually bond a little over their missing kids. It was Liz Taylor, who inspired Iris.

At the end of the episode, Alex returns to the Countess and consents to be turned, then drinks her blood. “I matter!” she screamed. Waking up with Sally next to him in bed. However, Lowe is confused on how it all went down. “It all starts with that lead actor, which [Lady Gaga] is for us, and she doesn’t not disappoint – I’ll tell you that much”. However, Iris worries that the Countess will know she’s been turned, and fears for her life, well, afterlife.
